APG-1387 + gemcitabine + nab-paclitaxel is an investigational combination regimen for advanced pancreatic cancer, combining three agents: APG-1387 (a novel small molecule SMAC mimetic and antagonist of inhibitor of apoptosis proteins, IAPs), gemcitabine (a nucleoside analog antimetabolite chemotherapeutic), and nab-paclitaxel (albumin-bound paclitaxel, a microtubule stabilizer). APG-1387, developed by Ascentage Pharma, promotes apoptosis in tumor cells by degrading cIAP1/2 and XIAP to sensitize tumors to cytotoxic agents. Gemcitabine inhibits DNA synthesis to induce cytotoxicity, while nab-paclitaxel disrupts microtubule function to arrest cell division. The combination aims to enhance anticancer efficacy and overcome chemo-resistance by simultaneously activating pro-apoptotic pathways and blocking tumor proliferation. This combination is currently being studied in phase Ib/II trials for advanced pancreatic cancer[3][5].
