Overview
Apremilast + dupilumab is a combination therapy investigated for the treatment of moderate-to-severe atopic dermatitis, particularly in patients with an inadequate response to dupilumab monotherapy. Apremilast is an oral small molecule inhibitor of phosphodiesterase 4 (PDE4), which increases intracellular cAMP and modulates the expression of multiple pro-inflammatory and anti-inflammatory mediators, including TNF-alpha, IL-12, IL-2, and others. Dupilumab is a fully human monoclonal antibody that targets the interleukin 4 receptor alpha subunit (IL‑4Rα), thereby inhibiting signaling from both IL‑4 and IL‑13 cytokines central to type 2 inflammation. The rationale for combining these agents is to address both type 2 immune pathways (via dupilumab) and additional inflammatory pathways such as Th1/Th17-mediated inflammation (via apremilast). This combination has been studied in open-label phase 2 trials for recalcitrant atopic dermatitis[2][3][5][7][8].
