apremilast + methotrexate is a combination therapy consisting of the phosphodiesterase-4 (PDE4) inhibitor apremilast and the folate antimetabolite methotrexate. This combination is being investigated in clinical trials, such as those conducted by Tanta University, for the treatment of psoriatic arthritis. Apremilast works by inhibiting PDE4, which increases intracellular cyclic adenosine monophosphate (cAMP) levels, thereby modulating the expression of various pro-inflammatory and anti-inflammatory mediators. Methotrexate acts by inhibiting dihydrofolate reductase (DHFR) and other folate-dependent enzymes, leading to the inhibition of DNA synthesis and cellular proliferation, as well as exerting anti-inflammatory effects through the release of adenosine. The combination aims to provide synergistic or additive therapeutic benefits in managing the symptoms and progression of psoriatic arthritis.
