APVAC1 vaccine + poly-ICLC + GM-CSF is a combination immunotherapy regimen developed for the treatment of newly diagnosed glioblastoma. APVAC1 is an actively personalized peptide vaccine composed of up to seven non-mutated HLA class I peptides, two pan-HLA class II-restricted tumor-associated peptides, and one HLA class I viral marker peptide, selected from a premanufactured library based on individual patient tumor profiling. Poly-ICLC (Hiltonol) acts as an immune adjuvant by mimicking viral double-stranded RNA and stimulating innate immunity through induction of type I interferons and pro-inflammatory cytokines via TLR3 and cytosolic receptors. GM-CSF serves as another adjuvant to enhance antigen presentation by recruiting and activating dendritic cells at the vaccination site. The combination aims to elicit robust central memory CD8+ T cell responses against unmutated tumor antigens, with demonstrated safety and immunogenicity in phase I clinical trials for glioblastoma[2][4][5][7].
