Overview

APVO436 + cytarabine is a **combination therapy** consisting of APVO436, a recombinant T cell-engaging humanized bispecific antibody, and cytarabine, a pyrimidine nucleoside antimetabolite chemotherapeutic agent. - **APVO436** targets CD123 (present on leukemic cells and leukemic stem cells) and CD3 (on T cells), redirecting the patient's T cells to CD123-expressing leukemia cells to induce targeted cell lysis in an MHC-independent manner. This represents a form of T cell-engaging immunotherapy for hematologic malignancies, notably acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS)[1][2][3][5]. - **Cytarabine** is an antimetabolite that inhibits DNA synthesis and is a foundational chemotherapy for AML, acting as a cytotoxic agent to proliferating leukemic cells. This combination is being investigated as a way to leverage immune-mediated cytotoxicity (via APVO436) together with direct cytotoxic chemotherapy (via cytarabine) to improve outcomes for patients with relapsed/refractory or high-risk AML. The combination is currently under clinical evaluation and is not yet approved for marketing. **APVO436** is being developed by Aptevo Therapeutics. The combination is being explored in newly diagnosed and relapsed/refractory AML patients, particularly those expressing CD123.
