Overview

APX005M is a humanized IgG1κ monoclonal antibody that acts as a potent agonist of CD40, an immune checkpoint protein expressed on antigen-presenting cells. By binding to specific epitopes on CD40 and blocking the interaction with its ligand (CD40L), APX005M activates the CD40 signaling pathway. This leads to activation of antigen-presenting cells (APCs), increased expression of co-stimulatory molecules (CD80, CD83, CD86), cytokine release from dendritic cells and lymphocytes, and enhanced T cell proliferation—resulting in robust innate and adaptive anti-tumor immune responses[1][2][5]. It has been engineered for high agonistic activity while minimizing antibody-dependent cellular cytotoxicity (ADCC)[1]. The drug is under development for various cancers both as monotherapy and in combination regimens. mFOLFOX refers to a modified FOLFOX chemotherapy regimen consisting of folinic acid (leucovorin), fluorouracil (5-FU), and oxaliplatin. This regimen is widely used for colorectal cancer treatment; it works by destroying rapidly dividing cancer cells through DNA damage or inhibition of DNA synthesis[6][8]. Radiation therapy uses ionizing radiation to kill or damage cancer cells. The combination of APX005M with mFOLFOX chemotherapy and radiation therapy is being investigated as a multi-modal approach for treating certain cancers such as gastrointestinal malignancies[7].
