ARB + CCB + ACE-inhibitor + statin + nitrate + antiplatelet-Yonsei University

This combination regimen, referred to as Optimal Medical Therapy (OMT), was utilized as the control arm in clinical trials conducted by Yonsei University, most notably the BRAVE study. The regimen is designed for the comprehensive management of coronary artery disease (CAD) and angina pectoris, particularly in elderly patients. It consists of a multi-drug approach including antiplatelet agents (such as aspirin), statins for lipid management, nitrates (like isosorbide nitrate) for vasodilation, calcium channel blockers (CCBs), beta-blockers, and either angiotensin-converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARBs) for blood pressure and neurohormonal control. The therapy targets multiple pathophysiological mechanisms, including platelet aggregation, lipid metabolism, vascular tone, and the renin-angiotensin-aldosterone system, to reduce major adverse cardiovascular events.
