Drug intelligence / Profile preview
**Arikayce** is a liposomal inhaled formulation of the aminoglycoside antibiotic amikacin developed by Insmed for difficult-to-treat nontuberculous mycobacterial lung disease caused by *Mycobacterium avium* complex. It is administered once daily via nebulization as part of a combination antibacterial regimen in adults with refractory disease and limited treatment options. The liposomal formulation is designed to deliver amikacin directly to the lungs and into infected macrophages and biofilm-associated environments, where amikacin exerts concentration-dependent antibacterial activity by binding the bacterial 30S ribosomal subunit and inhibiting protein synthesis. The product received accelerated approval in the United States in 2018 under the LPAD pathway and is marketed for refractory MAC lung disease.
