Arikayce (amikacin liposome inhalation suspension) is an inhaled aminoglycoside antibiotic developed by Insmed Incorporated for the treatment of Mycobacterium avium complex (MAC) lung disease. It is intended for use as part of a multi-drug regimen, specifically for adult patients with refractory disease who have limited or no alternative treatment options. The drug utilizes Insmed's proprietary Pulmovance liposomal technology, which encapsulates amikacin in liposomes to facilitate targeted delivery to the lungs via the Lamira Nebulizer System. This approach allows the drug to be taken up by alveolar macrophages, the primary intracellular site of MAC infection, thereby maximizing local concentration while minimizing systemic exposure and associated aminoglycoside toxicities (such as nephrotoxicity and ototoxicity). Amikacin exerts its antibacterial effect by binding to the bacterial 30S ribosomal subunit, inhibiting protein synthesis and disrupting cell membrane integrity. Arikayce received accelerated approval from the FDA in 2018 based on results from the Phase 3 CONVERT trial.
