Arsenic trioxide + ascorbic acid + dexamethasone + thalidomide is a combination therapy regimen specifically developed and investigated by The Cleveland Clinic for the treatment of multiple myeloma. In this protocol, ascorbic acid (vitamin C) is administered intravenously to act as a biochemical modulator. Its primary role is to deplete intracellular levels of glutathione, a major antioxidant that can neutralize the effects of arsenic trioxide. By reducing glutathione levels, ascorbic acid potentiates the pro-apoptotic and cytotoxic activity of arsenic trioxide against myeloma cells. The regimen also includes thalidomide, an immunomodulatory drug (IMiD) that targets the cereblon E3 ligase complex, and dexamethasone, a glucocorticoid that provides synergistic anti-tumor and anti-inflammatory effects. This four-drug combination has been evaluated in Phase II clinical trials for patients with previously untreated high-risk or relapsed/refractory multiple myeloma.
