Overview

This is a combination antimalarial regimen comprising: - Artemisinin-based combination therapy (ACT): A treatment for uncomplicated malaria that combines a fast-acting artemisinin derivative (such as artesunate, artemether, or dihydroartemisinin) with a longer-acting partner drug (e.g., lumefantrine, mefloquine, amodiaquine, piperaquine). The ACT rapidly reduces parasite biomass and the partner drug clears remaining parasites to ensure cure and delay resistance development[1][2][3]. - Injectable artesunate: Used as first-line treatment for severe malaria. Artesunate is an artemisinin derivative administered intravenously or intramuscularly; it acts rapidly to reduce parasite load in life-threatening cases[7][10]. - Rectal artesunate: A rectally administered formulation of artesunate used as pre-referral intervention in suspected severe malaria when parenteral administration is not possible. It provides rapid reduction of parasite biomass and buys time until definitive care can be given[7][10]. Mechanism of action involves activation by heme/iron within infected erythrocytes leading to production of free radicals that damage malarial proteins and membranes. This results in rapid killing of Plasmodium parasites at multiple life cycle stages[5][6]. The regimen targets both uncomplicated and severe malaria due to Plasmodium falciparum.
