Artesunate + sulfamethoxypyrazine + pyrimethamine is a fixed-dose combination antimalarial therapy used for the curative treatment of all forms of malaria, including severe malaria caused by multidrug-resistant strains of *Plasmodium falciparum*. Artesunate is an artemisinin derivative that rapidly kills parasites by generating free radicals through its active metabolite dihydroartemisinin, which disrupts parasite proteins and nucleic acid synthesis. Sulfamethoxypyrazine and pyrimethamine act synergistically as antifolates; sulfamethoxypyrazine inhibits dihydropteroate synthase, while pyrimethamine inhibits dihydrofolate reductase in the parasite, blocking folate synthesis essential for DNA replication. The combination provides both rapid reduction in parasitemia (via artesunate) and sustained suppression to prevent recrudescence (via antifolates). It is administered orally as tablets over three consecutive doses within 48 hours[1][5].
