Ascorbic acid (Vitamin C) and alpha-tocopherol (Vitamin E) are being evaluated as a combination antioxidant therapy by the University of Alabama at Birmingham for the prevention of coagulopathy and nosocomial pneumonia in patients following severe trauma. Traumatic injury often leads to significant oxidative stress, which can trigger systemic inflammatory responses and organ dysfunction. Ascorbic acid acts as a potent water-soluble antioxidant that scavenges reactive oxygen species (ROS) in the cytosol and extracellular fluid, while alpha-tocopherol serves as a lipid-soluble antioxidant that protects cell membranes from lipid peroxidation. This synergistic combination is intended to mitigate oxidative damage and improve clinical outcomes in critically injured patients. A Phase 2 clinical trial (NCT00182728) was conducted to assess the efficacy of this regimen in reducing the incidence of organ failure and infectious complications in the intensive care unit setting.
