This combination therapy, often referred to as the MOV regimen, consists of the biguanide metformin, the proton pump inhibitor omeprazole, and the antioxidant/pro-oxidant ascorbic acid (Vitamin C). Developed by The First Affiliated Hospital of Sun Yat-Sen University, it is being investigated for the treatment of advanced solid tumors. The rationale for this combination is based on a multi-targeted approach to cancer metabolism and the tumor microenvironment. Metformin acts by inhibiting mitochondrial complex I and activating AMPK, thereby disrupting cancer cell energy production. Omeprazole is utilized to inhibit V-type ATPases, which helps neutralize the acidic extracellular pH of the tumor microenvironment that typically promotes tumor invasion and drug resistance. Ascorbic acid, particularly at the high doses used in clinical trials, can act as a pro-oxidant to induce metabolic stress and oxidative damage in cancer cells. This regimen is currently being evaluated in Phase 2 clinical trials.
