ASCT + anti-CD19 CAR-T cells is a combination therapeutic regimen being developed by The Affiliated Hospital of Xuzhou Medical University for the treatment of B-cell non-Hodgkin lymphoma. The protocol involves high-dose chemotherapy conditioning (typically the BEAM regimen: carmustine, etoposide, cytarabine, and melphalan) followed by autologous stem cell transplantation (ASCT) to rescue hematopoiesis. Subsequently, patients receive an infusion of autologous T cells that have been genetically engineered to express a chimeric antigen receptor (CAR) targeting the CD19 antigen, which is expressed on the surface of most B-cell malignancies. This sequential approach aims to utilize the debulking and lymphodepleting effects of ASCT to create a favorable environment for CAR-T cell expansion and persistence, particularly in patients who have achieved only a partial response to initial first-line chemotherapy.
