ALK solution is an experimental intravenous metabolic support therapy consisting of a combination of L-aspartate, L-leucine, and alpha-ketoglutarate. It was developed to provide metabolic substrate support during high-stress surgical procedures, specifically abdominal aortic aneurysm (AAA) repair, where aortic clamping leads to ischemia-reperfusion injury. The components are intended to support the citric acid cycle (TCA cycle) and cellular energy production; aspartate and alpha-ketoglutarate serve as TCA cycle intermediates and facilitate the malate-aspartate shuttle, while leucine provides an oxidative fuel source and supports protein metabolism. The solution was evaluated in clinical trials, such as those conducted by the University of Leicester and funded by the UK Department of Health, to assess its efficacy in improving perioperative metabolic stability and glycemic control compared to glucose-insulin-potassium (GIK) or normal saline.
