Aspirin + clopidogrel is a fixed-dose combination of two antiplatelet agents used primarily to prevent heart attack and stroke in patients with acute coronary syndrome or those at high risk for cardiovascular events. Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1), reducing thromboxane A2 synthesis and thereby decreasing platelet activation and aggregation. Clopidogrel is a prodrug that irreversibly inhibits the P2Y12 subtype of adenosine diphosphate (ADP) receptor on platelets, preventing ADP-mediated platelet activation and aggregation. The combination provides greater efficacy in reducing major vascular events compared to either agent alone but increases the risk of bleeding. It is indicated for use after certain cardiac procedures, such as percutaneous coronary intervention (PCI), or in acute myocardial infarction, especially when revascularization is performed[1][4][5][6].
