aspirin + rivaroxaban

A combination therapy consisting of the antiplatelet agent aspirin (acetylsalicylic acid) and the direct oral anticoagulant rivaroxaban. This combination was investigated in the ARIVA trial, sponsored by the University of Zurich, for the prevention of early venous stent thrombosis in patients with post-thrombotic syndrome (PTS) following endovascular therapy. Aspirin acts as a small molecule inhibitor of cyclooxygenase-1 (COX-1), preventing the synthesis of thromboxane A2 and subsequent platelet aggregation. Rivaroxaban is a highly selective Factor Xa inhibitor that blocks the conversion of prothrombin to thrombin, thereby inhibiting clot formation. The ARIVA trial compared a regimen of 100 mg aspirin plus 20 mg rivaroxaban daily against rivaroxaban monotherapy. Results indicated that the addition of aspirin did not provide a significant benefit in 6-month stent patency rates compared to rivaroxaban alone in this specific clinical context.
