ASTX727 + donor lymphocyte infusion is an investigational combination therapy being evaluated by the M.D. Anderson Cancer Center for the treatment of acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS) following allogeneic hematopoietic stem cell transplantation. The regimen consists of ASTX727, an oral fixed-dose combination of the hypomethylating agent decitabine and the cytidine deaminase inhibitor cedazuridine, administered alongside donor lymphocyte infusion (DLI). Decitabine functions by inhibiting DNA methyltransferase 1 (DNMT1), which leads to the hypomethylation of DNA and the reactivation of silenced tumor suppressor genes. Cedazuridine is included to inhibit the enzyme cytidine deaminase, preventing the rapid degradation of decitabine in the gastrointestinal tract and liver, thereby achieving systemic exposure comparable to intravenous administration. DLI is an adoptive immunotherapy that involves infusing T-lymphocytes from the original stem cell donor to enhance the graft-versus-leukemia (GVL) effect, aiming to eradicate residual disease and prevent relapse.
