Overview

AT-406 (also known as Xevinapant, SM-406, ARRY-334543, and Debio1143) is a potent, orally bioavailable small molecule that acts as a Smac mimetic and antagonist of inhibitor of apoptosis proteins (IAPs), including XIAP, cIAP1, and cIAP2. By binding to these proteins with high affinity (Ki values: 66.4 nM for XIAP; 1.9 nM for cIAP1; 5.1 nM for cIAP2), AT-406 promotes the activation of caspases and induces apoptosis in cancer cells[1][6][8]. It has demonstrated significant preclinical efficacy in various cancer models—particularly ovarian cancer—and can sensitize resistant tumor cells to conventional chemotherapies such as carboplatin[2][4][6]. Daunorubicin is an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II, leading to DNA damage and cell death. Cytarabine is a nucleoside analog that inhibits DNA synthesis by incorporating into DNA during replication. The combination of AT-406 with daunorubicin and cytarabine represents an investigational regimen designed to enhance pro-apoptotic signaling while simultaneously targeting DNA integrity in malignant cells.
