Overview

Atezolizumab + epirubicin is an investigational combination therapy consisting of the immune checkpoint inhibitor atezolizumab and the anthracycline chemotherapy agent epirubicin. Atezolizumab is a monoclonal antibody targeting programmed death-ligand 1 (PD-L1), thereby blocking its interaction with PD-1 and B7.1 receptors on T cells, which enhances anti-tumor immune responses. Epirubicin is an anthracycline that intercalates into DNA and inhibits topoisomerase II, leading to cytotoxicity in rapidly dividing cancer cells. This combination has been studied in neoadjuvant settings for HER2-positive early breast cancer as part of multi-drug regimens (often with trastuzumab and pertuzumab), aiming to leverage both immunogenic cell death from chemotherapy and immune activation from checkpoint inhibition[1][6]. The regimen has shown high pathological complete response rates in clinical trials.
