Overview

Atezolizumab + isatuximab is an investigational combination therapy consisting of two monoclonal antibodies: atezolizumab, a humanized IgG1 antibody targeting programmed death-ligand 1 (PD-L1), and isatuximab, an IgG1 monoclonal antibody targeting CD38. Atezolizumab blocks the interaction between PD-L1 and its receptors PD-1 and B7-1, thereby enhancing T-cell mediated immune responses against tumors. Isatuximab binds to CD38 on tumor cells and immune cells, inducing direct apoptosis as well as immune-mediated cytotoxicity through mechanisms such as antibody-dependent cell-mediated cytotoxicity (ADCC), phagocytosis (ADCP), complement-dependent cytotoxicity (CDC), and inhibition of CD38 enzymatic activity. The combination has been studied in phase I/II trials for advanced solid tumors including epithelial ovarian cancer, glioblastoma multiforme, hepatocellular carcinoma, squamous cell carcinoma of the head and neck, and metastatic colorectal cancer. While the regimen demonstrated acceptable safety profiles with efficient target engagement by both agents in the tumor microenvironment, it did not show significant clinical benefit over standard therapies in these settings[1][3][4].
