Atezolizumab plus nab-paclitaxel is a combination therapy comprising the programmed death-ligand 1 (PD-L1) targeting monoclonal antibody atezolizumab and the nanoparticle albumin-bound chemotherapy agent nab-paclitaxel. This regimen is primarily indicated for the first-line treatment of adult patients with PD-L1-positive metastatic triple-negative breast cancer (mTNBC). Atezolizumab functions as an immune checkpoint inhibitor by binding to PD-L1 on tumor cells and tumor-infiltrating immune cells, thereby preventing its interaction with PD-1 and B7.1 receptors and restoring T-cell mediated anti-tumor immunity. Nab-paclitaxel, a microtubule-stabilizing agent, complements this by inducing tumor cell death and potentially enhancing the immune response through the release of tumor antigens and induction of immunogenic cell death. The combination was evaluated in the pivotal IMpassion130 trial and further studied in real-world settings, such as the ANASTASE study conducted by Fondazione Policlinico Universitario Agostino Gemelli IRCCS in Italy.
