Atomoxetine + quazepam (DAW2022) is an experimental combination therapy being evaluated for the treatment of obstructive sleep apnea (OSA). The combination addresses two distinct pathophysiological mechanisms of OSA: upper airway muscle dysfunction and a low respiratory arousal threshold. **Atomoxetine** is a selective norepinephrine reuptake inhibitor (NRI) that increases norepinephrine levels, thereby stimulating the genioglossus and other upper airway dilator muscles to maintain airway patency during sleep. **Quazepam** (DAW2020) is a long-acting benzodiazepine hypnotic that increases the respiratory arousal threshold, preventing the frequent, premature awakenings (arousals) that contribute to breathing instability in patients with the 'low arousal threshold' phenotype. By combining these agents, the therapy aims to stabilize breathing and reduce the frequency of apneic events. This combination was developed and is being tested by researchers at **Brigham and Women's Hospital**.
