Atorvastatin + dexamethasone is a combination therapy consisting of atorvastatin, a statin that inhibits HMG-CoA reductase to lower cholesterol and exert anti-inflammatory effects, and dexamethasone, a synthetic glucocorticoid with potent anti-inflammatory and immunosuppressive properties. This combination has been investigated primarily for the treatment of chronic subdural hematoma (CSDH) and intracerebral hemorrhage (ICH), where it aims to reduce inflammation, promote angiogenesis, stabilize vascular endothelium, decrease hematoma volume, and improve neurological outcomes. Clinical studies have shown that the combined regimen is more effective than monotherapy with either agent in reducing hematoma size and improving functional recovery in both animal models and human patients[1][3][4][5][6]. The mechanism involves synergistic suppression of inflammatory cytokines (e.g., IL-6, IL-8), enhancement of endothelial tight junction proteins (e.g., ZO-1), upregulation of Krüppel-like factor 2 expression, reduction in vascular permeability markers such as MMP-9 and VEGF, modulation of immune cell populations (increase in regulatory T cells), attenuation of NF-κB signaling pathways, improved neural function prognosis, reduced mortality rates after brain injury or hemorrhage[3][4][5].
