Atropine + pralidoxime is a combination of two small-molecule drugs used as the standard-of-care treatment for acute organophosphate (OP) poisoning and nerve agent exposure. Atropine acts as a competitive muscarinic acetylcholine receptor antagonist, reversing the life-threatening cholinergic effects (such as bradycardia, excessive bronchial secretions, and bronchospasm) caused by the accumulation of acetylcholine. Pralidoxime is an oxime-class acetylcholinesterase reactivator that works by displacing the organophosphate molecule from the active site of the acetylcholinesterase enzyme, thereby restoring its ability to break down acetylcholine at both muscarinic and nicotinic sites. This dual approach addresses both the symptoms and the underlying biochemical inhibition caused by organophosphates.
