Aumolertinib + pemetrexed + carboplatin is a combination therapeutic regimen being evaluated for the treatment of treatment-naïve, advanced non-small cell lung cancer (NSCLC), specifically lung adenocarcinoma harboring EGFR mutations such as L858R or exon 19 deletions. The regimen consists of aumolertinib, a third-generation, irreversible epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI) that selectively targets sensitizing and T790M resistance mutations, combined with a chemotherapy doublet of pemetrexed (an antifolate antimetabolite) and carboplatin (a platinum-based DNA alkylating agent). This combination aims to enhance efficacy by providing simultaneous targeted inhibition and cytotoxic action, potentially overcoming the inferior prognosis associated with certain EGFR mutations like L858R compared to exon 19 deletions. It is currently being studied in the Phase II ACCEL trial, which utilizes a phased approach including an induction phase with aumolertinib monotherapy followed by a consolidation phase with the triplet combination.
