Developed by the Mario Negri Institute for Pharmacological Research, this autologous bone marrow-derived mesenchymal stromal cell (MSC) therapy is an investigational immunomodulatory treatment for kidney transplant recipients. The therapy involves the ex vivo expansion of a patient's own bone marrow MSCs, which are then intravenously infused—ideally one day prior to transplantation. The mechanism of action centers on dampening effector T-cell responses and promoting the expansion of FoxP3+ regulatory T cells (Tregs), thereby creating a pro-tolerogenic environment. This approach aims to facilitate the minimization or complete withdrawal of maintenance immunosuppressive therapy while preserving long-term allograft function and preventing chronic rejection.
