Overview

Autologous bone marrow-derived mesenchymal stromal cells (MSCs) in combination with everolimus is an experimental therapeutic regimen developed by Leiden University Medical Center (LUMC) for renal transplant recipients. The therapy involves the intravenous administration of ex vivo expanded autologous MSCs, which possess potent immunomodulatory, anti-inflammatory, and pro-regenerative properties. These cells are used in conjunction with everolimus, a small molecule inhibitor of the mechanistic target of rapamycin (mTOR), to provide maintenance immunosuppression. The primary clinical objective of this combination is to facilitate the withdrawal of tacrolimus, thereby reducing calcineurin inhibitor-induced nephrotoxicity and renal fibrosis while maintaining graft stability and reducing the risk of opportunistic infections.
