Overview

Autologous bone marrow mononuclear cells (BM-MNCs) are a heterogeneous population of cells, including hematopoietic stem cells, mesenchymal stem cells, and endothelial progenitor cells, harvested from a patient's own bone marrow. In the ReNEW clinical trial led by Duke University, these cells are processed and delivered via intracoronary injection to patients who have recently experienced an acute myocardial infarction (AMI) and are at high risk for developing heart failure. The therapeutic objective is to promote myocardial salvage and improve cardiac healing. The mechanism is thought to involve paracrine signaling where the injected cells release growth factors and cytokines that stimulate angiogenesis, reduce apoptosis of cardiomyocytes, and modulate the inflammatory environment, thereby preserving left ventricular function and preventing adverse remodeling.
