Autologous CD133+ bone marrow-derived stem cells are an investigational cell therapy developed for the treatment of chronic ischemic cardiomyopathy. The therapy involves harvesting a patient's own bone marrow and enriching it for CD133-positive cells, which represent a population of primitive hematopoietic and endothelial progenitor cells with high regenerative potential. These cells are intended to be delivered to the ischemic myocardium—either via intracoronary infusion or direct intramyocardial injection—to stimulate neovascularization and improve cardiac function. The therapeutic effect is primarily mediated through paracrine signaling, where the cells secrete various growth factors and cytokines that promote angiogenesis, reduce apoptosis, and potentially facilitate myocardial repair. The AlsterMACS pilot study, conducted by Asklepios proresearch, specifically compared these two delivery routes to determine the most effective method for improving left ventricular ejection fraction in patients with chronic heart failure.
