Overview

Autologous CD34+ enriched cell fraction is an investigational cell therapy consisting of hematopoietic stem and progenitor cells (HSPCs) harvested from a patient's own peripheral blood or bone marrow. Developed by the Unidade Local de Saúde de Coimbra (ULS Coimbra), this therapy is specifically being evaluated for the treatment of acute ischemic stroke. The process involves mobilizing CD34+ cells using granulocyte colony-stimulating factor (G-CSF), followed by enrichment via immunomagnetic selection. The enriched cell fraction is then administered intra-arterially into the brain's ischemic territory. The therapeutic effect is mediated through paracrine mechanisms, where the cells secrete angiogenic and neurotrophic factors—such as vascular endothelial growth factor (VEGF) and insulin-like growth factor 1 (IGF-1)—that promote neovascularization, reduce neuronal apoptosis, and enhance endogenous repair processes in the ischemic penumbra.
