An autologous CD34+ stem cell boost is a cellular therapy procedure involving the reinfusion of a patient's own previously collected and cryopreserved CD34+ hematopoietic stem and progenitor cells. This intervention is primarily utilized as a salvage strategy to manage severe or prolonged immune effector cell-associated hematotoxicity (ICAHT), characterized by persistent cytopenias following B-cell maturation antigen (BCMA)-directed CAR T-cell therapy or bispecific antibody therapy in patients with relapsed/refractory multiple myeloma. The reinfused cells migrate to the bone marrow to promote multilineage hematopoietic recovery, effectively restoring counts of neutrophils, platelets, and red blood cells. This procedure is supported by guidelines from the European Society for Blood and Marrow Transplantation (EBMT) and the European Hematology Association (EHA).
