Overview

This autologous CD34+ stem cell therapy, developed by the University Medical Centre Ljubljana, is a cell-based biologic treatment for patients with nonischemic dilated cardiomyopathy (DCM) and chronic heart failure. The therapeutic process involves the mobilization of the patient's own CD34+ hematopoietic stem and progenitor cells from the bone marrow into the peripheral blood using granulocyte colony-stimulating factor (G-CSF/filgrastim). These cells are then collected via apheresis and purified. The resulting cell product is delivered directly to the heart, typically through NOGA-guided transendocardial (intramyocardial) injection or intracoronary infusion. The mechanism of action is primarily attributed to the promotion of neoangiogenesis and the release of paracrine factors that improve myocardial perfusion and viability in failing heart tissue. This therapy has been evaluated in several clinical trials, including the REMEDIUM study, which investigated the efficacy of repetitive dosing.
