A multi-component therapeutic cancer vaccine regimen composed of three elements: - **Autologous DNP-modified melanoma vaccine**: Tumor cells derived from the patient (autologous), chemically modified with the hapten dinitrophenyl (DNP) to enhance immunogenicity. - **Cyclophosphamide**: Low-dose chemotherapy administered as an immunomodulator to suppress regulatory T cells and potentiate the vaccine effect. - **BCG (Bacille Calmette–Guérin)**: A bacterial adjuvant, mixed with the vaccine to further stimulate local immune response. This regimen is used in the post-surgical (adjuvant) or metastatic melanoma setting. The vaccine’s mechanism involves stimulating delayed-type hypersensitivity (DTH) and activating T cell-mediated anti-tumor responses to melanoma antigens. Clinical studies suggest a correlation between robust DTH to unmodified melanoma cells and improved survival in melanoma patients. The regimen was primarily developed and clinically tested by academic centers and AVAX Technologies, but it has not received marketing approval due to regulatory and manufacturing challenges[1][2][3][4].
