Overview

Autologous fecal microbiota transplantation (FMT) is an experimental microbial therapy being developed by the Rambam Health Care Campus for the treatment of acute steroid-resistant gastrointestinal graft-versus-host disease (GI-GVHD). The procedure involves the collection and cryopreservation of a patient's own fecal matter prior to undergoing allogeneic hematopoietic stem cell transplantation (Allo-SCT). If the patient subsequently develops severe GI-GVHD, the stored autologous sample is processed into a standardized suspension and re-introduced via a nasogastric tube. This approach aims to restore the patient's baseline gut microbial diversity and composition, which are typically decimated by conditioning chemotherapy and broad-spectrum antibiotics, thereby mitigating the inflammatory response and promoting mucosal healing without the infectious risks associated with third-party donor FMT.
