Autologous hematopoietic stem cells (HSCs) are being utilized in a specialized therapeutic protocol developed by Cedars-Sinai Medical Center for the treatment of refractory Crohn's disease. This cell therapy involves a multi-step process: first, the patient's own stem cells are mobilized and collected from the peripheral blood; second, the patient undergoes a conditioning regimen of high-dose immunoablation (typically using cyclophosphamide, fludarabine, and anti-thymocyte globulin) to deplete the existing, dysfunctional immune system; and finally, the harvested HSCs are re-infused. The therapeutic goal is to reset the immune system, allowing for the development of a new, self-tolerant immune repertoire that can lead to long-term clinical remission in patients who have failed standard biologic and immunosuppressive therapies.
