Overview

Autologous hematopoietic stem cell transplantation (AHSCT) is a cellular therapy being investigated by the Shanghai Jiao Tong University School of Medicine for the treatment of newly diagnosed type 1 diabetes mellitus. The therapeutic approach involves the mobilization of a patient's own hematopoietic stem cells using granulocyte colony-stimulating factor (G-CSF), followed by their collection and cryopreservation. The patient then undergoes a nonmyeloablative conditioning regimen, typically consisting of high-dose cyclophosphamide and antithymocyte globulin (ATG), to deplete the autoreactive T-cells responsible for the destruction of pancreatic beta cells. The harvested stem cells are subsequently reinfused to reconstitute a reset immune system, with the goal of inducing long-term remission and preserving endogenous insulin production as measured by C-peptide levels.
