Overview

Autologous peripheral blood mononuclear cell (PBMNC) therapy is a biologic treatment developed by Mahidol University's Faculty of Medicine Siriraj Hospital for the treatment of chronic critical limb ischemia (CLI). The therapy involves the mobilization of a patient's own PBMNCs using granulocyte colony-stimulating factor (G-CSF) at doses of 5-10 µg/kg/day for three days. Following mobilization, the cells are collected via apheresis using a blood cell separator. The resulting cell product, which is enriched with CD34+ hematopoietic stem and progenitor cells, is then administered via multiple intramuscular injections into the gastrocnemius muscle of the ischemic limb. This cell therapy is designed to promote therapeutic angiogenesis and neovascularization, thereby improving blood perfusion and clinical outcomes in patients with "no-option" CLI who are not candidates for traditional surgical bypass or percutaneous transluminal angioplasty.
