Overview

Autologous peripheral blood stem cell transplantation (PBSCT) with in vivo purging is a specialized therapeutic regimen developed by the King Faisal Specialist Hospital & Research Center for the treatment of acute myeloid leukemia (AML). This approach is primarily utilized for patients in their first or second complete remission who lack a human leukocyte antigen (HLA)-matched related donor. The process involves 'in vivo purging,' where chemotherapy or targeted agents are administered to the patient to eliminate residual leukemic cells from the peripheral blood prior to the collection of hematopoietic stem cells. These 'purged' cells are then harvested, cryopreserved, and subsequently re-infused into the patient following high-dose conditioning chemotherapy. The goal of this strategy is to provide a curative hematopoietic reconstitution using the patient's own cells while minimizing the risk of leukemic relapse and avoiding the graft-versus-host disease (GVHD) typically associated with allogeneic transplants.
