Autologous peripheral blood stem cell transplantation (PBSCT) with in vivo purging is a cellular therapy protocol developed by the King Faisal Specialist Hospital & Research Center specifically for patients with Acute Myeloid Leukemia (AML). This therapeutic approach is designed for adult patients in their first or second complete remission who lack a human leukocyte antigen (HLA)-matched related donor. The process involves the mobilization and collection of the patient's own hematopoietic stem cells from the peripheral blood. The 'in vivo purging' component involves administering treatment to the patient prior to stem cell harvest to eliminate residual leukemic cells from the circulation, thereby reducing the risk of re-infusing malignant cells. Following high-dose conditioning chemotherapy intended to eradicate any remaining disease, the harvested stem cells are re-infused into the patient to restore bone marrow function and hematopoietic recovery.
