Autologous platelet-rich plasma (PRP) and fibrin clot is an investigational regenerative biologic preparation produced using the PRGF-System (Plasma Rich in Growth Factors) technology. The product is derived from the patient's own peripheral blood, which is processed to concentrate platelets and create a fibrin scaffold. When applied locally to complex perianal fistulas, particularly in patients with Crohn's disease, the preparation releases a concentrated pool of endogenous growth factors, including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF). These factors act on their respective receptors to stimulate angiogenesis, fibroblast proliferation, and extracellular matrix synthesis, thereby promoting tissue repair and wound healing while modulating the local inflammatory environment. The treatment is currently being evaluated in Phase II clinical trials by Fundación FISEVI in Spain for patients who have not responded to standard therapies.
