This autologous cell therapy was developed and investigated by researchers at the **Johann Wolfgang Goethe University Hospital** in Frankfurt, Germany. The treatment involves the isolation and intracoronary administration of the patient's own (autologous) progenitor cells, typically derived from either **bone marrow mononuclear cells (BMMNCs)** or **circulating blood-derived progenitor cells (CPCs)**. In the case of blood-derived cells, they are typically cultivated ex vivo for several days to enrich for endothelial progenitor cells (EPCs) before infusion. The therapy is designed to treat **acute myocardial infarction (AMI)** and **chronic ischemic coronary artery disease (CAD)** by promoting myocardial regeneration, enhancing neovascularization (angiogenesis), and improving left ventricular function through paracrine effects and potential cell integration. The clinical development was highlighted by the **TOPCARE-AMI** and **TOPCARE-CHD** trials, which were among the first randomized studies to demonstrate the safety and potential efficacy of intracoronary cell transplantation in humans.
