Autologous tumor-infiltrating lymphocytes (TILs) is an adoptive cell therapy being developed by the Centre Hospitalier Universitaire Vaudois (CHUV) for the treatment of advanced or metastatic solid tumors. The therapeutic process involves extracting naturally occurring T cells from a patient's tumor, expanding them ex vivo to large numbers, and re-infusing them into the patient. In the specific clinical protocol (NCT04100291), the TIL infusion is preceded by lymphodepleting chemotherapy (cyclophosphamide and fludarabine) and low-dose irradiation (LDI) to the tumor lesions to enhance the engraftment and efficacy of the transferred cells. Post-infusion, patients may receive high-dose interleukin-2 (IL-2) to support the survival and expansion of the TILs in vivo. This approach leverages the diverse T-cell receptor (TCR) repertoire of TILs to target a broad range of tumor-specific neoantigens and tumor-associated antigens.
