Autologous tumor-infiltrating lymphocyte (TIL) therapy is an investigational adoptive cell immunotherapy being developed by Inge Marie Svane at the Center for Cancer Immune Therapy (CCIT), Herlev Hospital, for the treatment of metastatic renal cell carcinoma. The therapeutic process involves the surgical resection of a tumor lesion from the patient, from which naturally occurring T cells (TILs) are isolated and expanded ex vivo to billions of cells over a period of 4 to 6 weeks. Prior to the infusion of these expanded TILs, patients receive a lymphodepleting conditioning regimen consisting of cyclophosphamide and fludarabine to create a favorable environment for T-cell engraftment. Following infusion, high-dose interleukin-2 (IL-2) is administered to support the activation, proliferation, and persistence of the TILs in vivo. This approach aims to utilize the patient's own immune system to recognize and destroy cancer cells by targeting a broad spectrum of tumor-specific antigens and neoantigens.
