Overview

This autologous tumor-infiltrating lymphocyte (TIL) therapy is a personalized cellular immunotherapy being developed by the H. Lee Moffitt Cancer Center and Research Institute, in collaboration with Johns Hopkins All Children's Hospital, for the treatment of high-risk pediatric malignant solid tumors. The therapeutic process involves the surgical resection of a patient's tumor to harvest naturally occurring TILs, which are then expanded ex vivo to approximately one billion cells at Moffitt's Cell Therapy Facility. Following a lymphodepleting conditioning regimen of fludarabine and cyclophosphamide, the expanded TILs are reinfused into the patient, followed by the administration of interleukin-2 (IL-2) to promote T-cell persistence and activity. This approach leverages the patient's own immune system to recognize and attack a broad spectrum of patient-specific tumor antigens, aiming to provide a durable response in refractory pediatric cancers such as neuroblastoma, osteosarcoma, and rhabdomyosarcoma.
