Autologous tumor-infiltrating lymphocyte (TIL) therapy is an adoptive cell-based immunotherapy developed by Sheba Medical Center for the treatment of metastatic ovarian cancer. The therapeutic process involves harvesting T-cells directly from a patient's surgical tumor specimen, selecting those with the highest tumor-reactive potential, and expanding them ex vivo using the stimulatory cytokine interleukin-2 (IL-2). Prior to re-infusion, patients undergo a non-myeloablative lymphodepleting regimen, which in this specific protocol includes fludarabine and low-dose total body irradiation. The expanded TILs are then re-infused into the patient, followed by high-dose IL-2 administration to support the survival and proliferation of the transferred cells. This approach leverages the patient's own immune system to recognize and attack diverse tumor-associated antigens present on metastatic lesions.
