Autologous tumor-infiltrating lymphocytes (TILs) are an adoptive cell therapy being developed by the Västra Götaland Region, specifically through researchers at Sahlgrenska University Hospital, for the treatment of metastatic melanoma. The therapy involves the surgical extraction of T cells that have naturally infiltrated a patient's tumor, followed by large-scale ex vivo expansion using interleukin-2 (IL-2). These expanded TILs, which possess endogenous T-cell receptors (TCRs) specific to the patient's unique tumor antigens, are then re-infused into the patient. In the GoTIL-01 clinical trial, the therapy is being evaluated via two administration routes: systemic intravenous infusion and locoregional hepatic arterial infusion (HAI), the latter specifically targeting liver metastases in uveal melanoma. The treatment protocol typically includes a lymphodepleting pre-conditioning regimen with melphalan and post-infusion support with low-dose IL-2 to promote T-cell persistence.
