This autologous tumor-infiltrating lymphocyte (TIL) therapy is a personalized adoptive cell transfer biologic developed by Yale University for patients with metastatic melanoma, specifically those who are refractory to PD-1/PD-L1 inhibitors. The treatment involves the surgical resection of a patient's tumor, from which naturally occurring TILs are extracted and expanded ex vivo to large therapeutic quantities at the Yale Advanced Cell Therapy Laboratories. Following a lymphodepleting chemotherapy regimen of cyclophosphamide and fludarabine, the expanded TILs are reinfused into the patient, followed by the administration of interleukin-2 (IL-2) to promote T-cell proliferation and persistence. The therapy utilizes the diverse, patient-specific T-cell receptor (TCR) repertoire of the TILs to recognize and eliminate tumor cells presenting various neoantigens.
