Overview
Tavneos (avacopan) in combination with cyclophosphamide is an immunosuppressive regimen used for the induction of remission in adult patients with severe active anti-neutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, specifically granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). Avacopan is an orally administered, small-molecule selective antagonist of the complement C5a receptor (C5aR), which blocks C5a-mediated neutrophil activation and migration. Cyclophosphamide is a nitrogen mustard alkylating agent that exerts potent immunosuppressive effects by cross-linking DNA and disrupting cell division in rapidly proliferating immune cells. This combination is administered as a co-administered regimen, typically followed by maintenance therapy with azathioprine or mycophenolate mofetil, and is designed to reduce the need for high-dose glucocorticoids.
